These links are for demonstration purposes and are not included in any data collection/reporting from the site. All of the data collected from patients is housed on a HIPAA compliant server and NONE of the data transmitted to the center within the downloaded reports contains any patient protected data. Downloaded data files are de-identified.
We are currently in Beta Testing to identify any operational issues with the survey. If you are interested in helping us with this process, all of the data that is collected will remain as part of your data collection when the final survey is launched. We encourage you to use the survey with all patient encounters and ask that you communicate any issues, problems, or suggestions that you have to improve it in the future.
