What to engrave on Medical Alert tag for LVAD recipients?

Hello, I have put off getting a medical alert tag for a while now, but now I want to travel more and spend more time enjoying my good days. I always want to be as prepared as possible because I know it helps if activities and possible scenarios are thought through.

I want to get a medical alert tag and have read they usually have room for 5 lines with 20 characters including spaces. The common set up is:

Line 1: Name

Line 2: medical condition or reason for tag

Line 3: allergies or special instructions

Line 4: med list or special medication

Line 5: ICE#. Usually Supporting Dr.

I would love to see suggestions from folk who have LVADs so I can make it as clear as possible. I am traveling to places where LVADs are practically unheard of and need my tag to be very concise.

That definitely looks cool. It's good that it is waterproof. I thought about the USB tag (although I never saw this one.... It's stylish), but I'm worried about going overseas and them being aware or capable or reading the USB device. However that would be a lifesaver to have around these parts. I guess I might be overthinking this...

I also have the Medical History Bracelet. I've had it almost a year now. Fortunately I haven't needed it but while I was in Cardiac Rehab after getting my VAD I showed it to the nurses there and they were all very impressed with it. The only downside I found is that the first time its plugged into a computer you may get a security warning especially if it's in a hospital where everything is protected. The auto run feature only seems to work after the USB has been set up the first time.

i am trying to find a bracelet for my son and not sure where to get it, help!
